Already on MassHealth? Redeterminations Are Getting Tougher

In my practice, I have been hearing more and more from clients who have been on MassHealth for years, who are suddenly being asked to provide mountains of documentation at renewal time. What used to be a relatively routine check-in has become something that looks and feels like a brand new application.


If you or a family member is currently receiving MassHealth benefits and has received a redetermination notice, please read this carefully.

What Is a Redetermination?

Redetermination is the process by which MassHealth periodically reviews whether you are still eligible for your benefits. MassHealth wants to confirm that your income, assets, and living situation still meet the program's requirements. Historically, redeterminations for seniors receiving long-term care benefits happened infrequently, and for many people, they were relatively simple. That has changed dramatically.


What We Are Seeing

Clients who have been on MassHealth for years, whether in a nursing home or living at home and receiving Community MassHealth benefits, are now being asked for five years' worth of bank statements. Not just a recent account summary. Five years of statements, across all accounts.


For many, this is more information than they were asked for when they applied initially. And along with those statements, MassHealth is now scrutinizing spousal assets far more carefully than in the past. Spouses who have been living independently for years, with assets that were previously not questioned, are now finding those assets front and center in the redetermination review. This can cause a problem if the spouse assets are now over the $162,000 limit.


For many families, this is shocking. They assumed that because benefits were approved years ago, the situation was settled. Unfortunately, eligibility is always ongoing, and MassHealth always has the right to review it.


What's Driving These Changes

Some of this reflects a broader national shift. The One Big Beautiful Bill, signed into federal law in 2025, requires states to conduct eligibility redeterminations every six months for certain Medicaid recipients. This is a dramatic increase from the prior annual schedule. While this provision is primarily aimed at the Medicaid expansion population (adults under 65), the ripple effects are being felt across MassHealth programs as the agency gears up for significantly more review activity. MassHealth is also facing its own budget pressures, which creates an additional incentive to review eligibility carefully.


What You Should Do Right Now

Whether or not you have received a redetermination notice, here is my advice:

Gather your records. Start collecting five years of bank and financial account statements for both you and your spouse. Having these organized and accessible will save enormous stress if a request arrives.

Don't ignore notices. MassHealth notices often have tight response deadlines. Missing a deadline can result in a loss of coverage. If you receive something that you don't understand, call us.

Don't respond alone if the situation is complex. If MassHealth is asking questions about asset transfers, gifts, or your spouse's finances, this is not a do-it-yourself situation. The wrong answer, or an incomplete answer, can create serious problems.
